How to Enroll:
Enrollment is a 3 step process!
1-Financial Aid Application (Complete at studentaid.gov Enid code 013879, Woodward code 016979)
2-School Tour & Application (Email enroll@formationsinstitute.edu to schedule tour)
3-Enroll (After financial aid application and school application are completed, matched, and verified, Formations Institute will scedule your enrollment appointment)

Our clinic floors are open to the public from 9:30AM - 3:00 PM Monday through Friday.
Some services require consultations in advance, so book accordingly!

Your appointments are very important to our Students, it is reserved especially for you, we understand that sometimes schedules adjustments are necessary; therefore, we respectfully request at least 24 hours notice for cancellations Please understand that when you forget or cancel your appointment without giving enough notice, we miss the opportunity to fill that appointment time, and clients on our waiting list miss the opportunity to receive services.

MISSION STATEMENT:
To provide students with a positive learning experience that will promote the skills needed to acquire licensure and gain the competencies necessary to compete as professional leaders in the industry. We lead students to fly in formation so they can position themselves to take full advantage of the updraft created by the learning and education they have received to soar ahead in the industry, and lead them to flap their wings at just the right time to maximize an updraft and minimize any downdraft!
